Medical directors must inform the Non-Hospital Medical and Surgical Facilities Accreditation Program of any reportable incident within one day of the incident being discovered. The timeline for submitting the reportable incident form and clinical chart has been extended to 30 days to ensure a complete document is submitted. This allows time for facilities to gather the dictated operative report, physician summary and any follow-up documentation from the hospital or physician office.
The complete clinical chart is no longer required to be submitted for return to the operating room for evacuation of hematomas in either non-hospital facilities or hospitals. The program may request the chart following review of the incident form.
The reporting of surgeries lasting longer than six hours has been updated to exclude complex aesthetic face and neck surgery.
