Appropriate procedures lists
The appropriate procedures list must be included with the application. It must be completed indicating only the procedures requested to be performed at the facility. The applicant must sign the list and review it with the medical director.
The NHMSFAP may require additional information if a procedure request is not listed below, or if the procedure or surgical service is not currently being performed at the facility. If this is the case, contact the NHMSFAP directly prior to submitting the application.
Appropriate procedures list(s) must be submitted to the facility’s medical director.
Once the medical staff has been granted privileges to perform the selected procedure(s), the medical director must email a copy of the appropriate procedures list(s) to nhmsfap@cpsbc.ca.
