Report a lost or stolen duplicate prescription pad
Each duplicate pad is personalized to an individual registrant and numerically tracked.
Drug programs staff can assist with finding the pad-specific information (e.g. folio number) that you will need to provide to the Ministry of Health for the PharmaNet FanOut.

Privacy breach and theft
If the lost/stolen pad contained patient-specific information (e.g. blue carbon copies retained in book), contact the at 1-800-663-7867 to report the potential privacy breach.
If the pad was stolen, consider contacting local law enforcement as well as the Canadian Medical Protective Association (CMPA) at 1-800-267-6522.
PharmaNet FanOut
Notifying all the pharmacies within a specified area
PharmaNet FanOut messages can be issued within one business day for lost or stolen prescription pads. This service is managed by the Ministry of Health.
Call PharmaNet Support Services at 1-844-660-3200
Provide the following information:
- prescriber’s name
- prescriber’s location
- prescriber’s licence number
- folio numbers (can be obtained by calling the drug programs at 604-733-7758 extension 2629)
- contact information
The PharmaNet Support Services phone line is answered during standard work hours (Monday to Friday, 8:30 a.m. to 4:30 p.m.). You can leave a voice mail message at any time with the information outlined above.
A person reporting a lost or stolen prescription pad must be a registrant of the ºÚÁÏÉç, the BC ºÚÁÏÉç of Oral Health Professionals, the BC ºÚÁÏÉç of Nurses and Midwives, or the ºÚÁÏÉç of Pharmacists of BC.
The following issues will not be communicated via FanOut, except in exceptional circumstances:
- prescription forgeries
- pharmacy robberies
- impersonation of a health-care provider or insurer to seek personal information
- multi-doctoring
- prescribers cancelling prescriptions
- lost prescriptions
- tracking missing persons or persons of interest in criminal investigation
PharmaNet Support Services may review critical information about issues other than lost or stolen prescription pads on a case-by-case basis.
Request a registrant prescribing profile
One of the best ways to determine if any unauthorized prescriptions have been dispensed is to request a registrant prescribing profile (RPP). This will allow a registrant to review all controlled prescriptions filled under their CPSID.
Request in writing to drugprograms@cpsbc.ca or via fax at 604-733-1267. Provide a fax number or address where the profile can be sent.
As this report contains confidential information, it can only be sent via fax or courier mail.
Steps for documenting forgery
- Gather copies of the forged prescription(s) from the pharmacy. If possible, email the copies to drugprograms@cpsbc.ca or fax 604-733-1267.
- Identify any discrepancies from legitimate prescriptions under your name.
- Document the forgery in your records.
- Collaborate with the pharmacy that received and/or filled the forged prescription(s).
- Share applicable details with the pharmacy staff so that they can identify possible future forgeries under your name.
- Direct the pharmacy staff on the steps you would like taken for suspected forgeries received under your name (e.g. how best to contact you for verification).
- Confirm who will notify, as appropriate, the local law enforcement.
